Where you live
can affect how long you live.
Traditionally, life expectancy is calculated by averaging the age of death in a neighborhood. In addition, research1,2,3 shows that factors present in the location you live—such as access to healthcare, education, nutritious food, and economic stability—also play a crucial role. Since people spend 90% of their lives within a 5-mile radius of their homes, their immediate environment has an outsized impact on their well-being—shaping everything from daily opportunities to long-term health outcomes.
When we compare life expectancy across different parts of the United States, the disparities are striking—even more so when they occur in neighborhoods just a few blocks apart. In one instance, communities separated by only 17 miles see a 16.84-year difference in life expectancy.4 In other cities, the gap can be as large as 30 years.5
To better understand what influences health and longevity, it’s important to take a closer look at the social determinants of health (SDOH)—the key factors that shape well-being across communities.
With a mission to level the healthcare playing field in underserved communities, Equality Health Foundation is dedicated to educating and supporting neighborhoods in improving their health and well-being. But the impact of health disparities extends far beyond medical outcomes—these inequities come with major financial consequences as well.
Take South Phoenix, for example. Research shows that babies born here may live up to 14 years less1 than those just a few miles away in North Scottsdale. This stark contrast highlights the urgent need for action. If disparities like these remain unaddressed, the financial burden will only grow—Deloitte projects that U.S. healthcare costs for the average American could triple by 2040.
To address these challenges, Equality Health Foundation is actively working in South Phoenix. Through a Well-Being Transformational Initiative in the South Central Corridor and surrounding areas, the foundation is leading efforts to improve health and quality of life.
To expand this work nationwide, Equality Health Foundation launched the ZIP Code Exam—which allows people across the United States to research life expectancy in their own communities. Through this platform, individuals can explore local health data and take action to improve their own health outcomes and those of their communities. Make sure to check out your ZIP Code statistics here.

Some ZIP Codes correspond to mailboxes rather than residential areas. Additionally, data is often provided at the Census Tract or ZCTA level—mathematical divisions of the country that don’t always align perfectly with ZIP Codes. Some regions also lack a designated ZIP Code, which may account for missing data in certain areas.
Life expectancy estimates are based on aggregated health and demographic data, offering a broad overview rather than a precise prediction for individuals. The CDC provides this data at the Census Tract level, and slight variations may occur when converted to ZIP Code data. For the most accurate and up-to-date information, we encourage you to visit the CDC website or check with your local government.
All data used on this website comes from public sources, including Census Tract Data (2009-2020), the Agency for Healthcare Research and Quality (Department of Health and Human Services) Social Determinants of Health Database, CDC Social Determinants of Health non-medical factors, and the Economic Research Service (U.S. Department of Agriculture). Walkability data is sourced from Walkscore.com. Please note that the latest available data on these platforms may reflect conditions from five or more years ago, and figures may vary between sources. While this is not meant to provide an exact snapshot, it aims to raise awareness about how your surroundings can impact your health and well-being—encouraging you to explore further and take action.
The factors used to evaluate health outcomes and generate each ZIP Code’s life expectancy number were chosen based on CDC’s Healthy People 2030 objectives, such as access to medical care and healthy food, completion of high school, and more. You can check out the full list of goals here, The score for each ZIP Code was also calculated based on the 0-100 system used by the CDC.
Keep in mind that life expectancy is influenced by many factors—not just the ones covered here—but also personal lifestyle and genetics. While the factors we highlight are systemic, there are ways to drive change. Visit our Take Action section for resources, community programs, and policy initiatives that can help improve health in your area.
This website serves as a starting point, not a final conclusion, for policymakers, researchers, and community leaders working to address health disparities. We encourage leaders to build on this data, conduct further research, and gather insights tailored to their communities.
The data presented is based on aggregated ZACT, ZIP Code and Census Tract statistics. For an easier visualization, it has been converted and standardized to ZIP Codes. While every effort has been made to ensure accuracy:

Social determinants of health, also known as SDOH, are the aspects of our surroundings that affect how healthy we are. They are the conditions in which we are born, live, learn, work, play, and grow older. These factors help decide how well we stay healthy and enjoy our lives.
Social determinants of health are the aspects of our surroundings that affect how healthy we are. They are the conditions in which we are born, live, learn, work, play, and grow older. These factors help decide how well we stay healthy and enjoy our lives. There are five main groups of social determinants:
People with low income or unstable employment often struggle to afford housing, food, healthcare, and education, which increases their risk of chronic disease and poor health outcomes.
Here are a few factors used to measure economic stability of certain ZIP Codes:
Limited access to quality education leads to lower health literacy, fewer job opportunities, and lower income, all of which contribute to poor long-term disadvantages when it comes to health.
These factors help assess education access and quality in your area:
People without health insurance, nearby medical facilities, or access to preventive care often suffer from undiagnosed and untreated conditions. This leads to more frequent emergency room visits and poorer health outcomes.
How can this be measured in your ZIP Code?
Unsafe housing, pollution, crime, lack of transportation, and food deserts directly affect physical and mental health.
The neighborhood and built environment of a ZIP Code can be assessed through several items:
Lack of social support, high levels of discrimination, and weak community engagement can contribute to chronic stress, poor mental health, and reduced access to resources.
The social and community context of a ZIP Code are measured through some key factors:

While structural issues are largely dependent on government policies and other stakeholders, there are things that can be done to start improving personal health outcomes, like accessing free online education, getting a free health screening, or applying for government aid. Here are some free resources:
